Too much damage
A tooth broken below the gum or split through the root often cannot be repaired.
Extractions, Portland
Sometimes a tooth cannot be saved. When that happens, how it comes out matters, because the bone around it is what a future implant will need. We do simple extractions here, protect the socket, and refer you to an oral surgeon when a case is complex.
The idea
An extraction should never be the end of the plan. It is the first step of the next one, and the bone it leaves behind is worth protecting.
Why
A tooth broken below the gum or split through the root often cannot be repaired.
When decay has destroyed too much of the tooth, or an infection cannot be cleared.
Severe gum disease can loosen a tooth until it cannot be kept.
Sometimes a tooth comes out as part of a larger plan, for example before a denture or implants.
The visit
We confirm the tooth cannot be saved, check the roots and the bone, and tell you whether we can do it here.
A local anaesthetic numbs the tooth and the gum. You stay awake and feel pressure, not pain.
The tooth is loosened and lifted out. We work slowly to protect the bone around it.
Where an implant is planned, a small graft may be placed in the socket to preserve the bone, and the gum is closed.
You go home with written instructions, pain relief advice and a call from us the next day.
Referral
We do simple extractions. If a tooth is impacted, if the roots are curved or fused to the bone, or if a larger graft or sinus work is part of the plan, we refer you to an oral surgeon and tell you why. We stay involved, and your plan shows how the two parts connect.
We do not claim a surgical specialty. We would rather send you to the right office than stretch what we offer. Results vary and depend on the exam.

The area is numbed, so you feel pressure and movement, not pain. Soreness afterwards is common and is usually managed with ordinary pain relief taken as directed.
Most people feel better within a few days, and the gum heals over a couple of weeks. We give written instructions and call you the next day to check.
Sometimes, but often the socket is allowed to heal first. The plan we write for you says which route suits your case.
A small amount of bone graft material is placed in the socket after the tooth comes out. It helps keep the bone from shrinking, which can make a future implant simpler.
Impacted and surgical cases are referred to an oral surgeon. We tell you why, and we stay involved in the overall plan.
We do not list prices on this site. You get a written estimate after the exam and before treatment, and we check what your insurance is expected to cover.
Portland, Oregon
An implant consultation is an exam, a 3D scan and a conversation. You leave with a written, phased plan and an estimate. Nothing is decided that day.
